PRP Case Manager

The case manager will be responsible for managing a caseload of minors and adult clients.
• Provide care coordination that includes developing daily living skills, increasing community integration, and helping clients meet rehabilitation goals (such as budgeting, medication compliance, housing, etc.
• Spend most of their time in the community meeting with clients one-on-one or onsite.
• Develop and maintain relationships with other providers in the community.
• Attend meetings and collaborate with treatment teams
• Complete daily contact notes and monthly reports quickly and accurately using provided device.

Education/Experience:

  • Bachelor’s degree in human services, arts, or social services with at least 1 year of experience working with clients with emotional or psychiatric diagnosis.

  • High school diploma with some coursework in social services or human services, with at least 3 years of experience in providing therapeutic or treatment services to clients with severe emotional or psychiatric diagnosis.

Qualifications:

  • Experience working in a behavioral healthcare setting.
  • A passion for working with individuals diagnosed with mental health disorders.
  • Knowledge of community-based organizations and resources.
  • Experience linking clients with mental health disorders to community resources.
  • Experience and comfort meeting clients in their home or in the community.
  • Strong time management and organization skills.
  • Maryland drivers license with a reliable and insured car.
  • Impeccable documentation skills and ability to collaborate with other care providers.
  • Familiar with treatment plans and goal setting.

Mental Health Therapists | Substance Abuse Counselors | Social Workers

Compassionate Wellness Center, LLC is a CARF accredited OMHC. Compassionate Wellness Center is currently seeking full- time, part- time, or contractual Mental Health and Substance Abuse Counselors and Social Workers to join our integrated outpatient mental health center. Our Comprehensive Treatment Center is unique in that we provide psychiatric rehabilitation for qualified patients.

Essential Duties and Responsibilities:

  • Perform initial assessment and follow up assessments.
  • Evaluates patient’s needs and determine additionally appropriate resources.
  • Conduct individual counseling individual, group, and/or family sessions and document per CWC policy.
  • Complete documentation of sessions per CWC policy.
  • Facilitate assigned group or family sessions.
  • Prepare individualized treatment plans for each assigned patient.
  • Ensures all documentation regarding patient care, treatment, and incidents is completed in a timely, clear, and concise manner.
  • Act as a liaison between referral sources and patients if need be.
  • Provides crisis intervention to patients as needed.
  • Complete discharge summaries.

Qualifications: LCADC, CAC, AD, CSC-AD, LMSW, LGPC.

LMSW – Must have completed a Masters Level Social Work program and successfully passed the licensing exam.

LGPC – Must have completed a Masters level Professional Counseling Degree and successfully passed the licensing exam.

LCADC – Has a master’s degree or doctoral degree in counseling or counseling-related health or human service field. Graduate degrees in other areas may be substituted if the candidate has completed an equivalent program with at least 51 documented counseling credits hours.

CAC-AD – Must have a bachelor’s degree in a qualifying health and human service field. At least 21 credit hours of coursework in drug and alcohol counseling and one hour in ethics. This is a supervised position (CWC provides supervision hours).

CSC-AD – Must have an associate’s degree and course work in substance abuse as prescribed by the Maryland Board of Substance Abuse Counselors. This is a supervised position (CWC provides supervision hours).

Psychiatric Nurse Practitioner

 

Essential Duties a& Responsibilities:

  • Assess patients’ needs and treat acute or chronic mental health disorders in an outpatient setting.
  • Prescribe and manage psychiatric medications and treatment regiments for clients with mental health and co-occurring disorders.
  • Monitor patient progress, adjust medications, and make necessary treatment recommendations and interventions.
  • Document all encounters with patients in the EMR.

    Education & Qualifications & Licensure:  Masters degree, CRNP-PMH certified, Maryland License, active DEA. MAT
